By signing below, you agree and acknowledge that Provider has not screened, diagnosed, or interacted with you or prescribed this Program for you, and your participation in the Program does not constitute medical or healthcare advice and does not create a provider-patient relationship between you and Provider. Please note that the Program is a prerecorded program and is not live. While you may have opportunity to message and receive responses from the Provider, such messaging service is provided by the hosting platform for the Program and is not confidential or subject to the Health Insurance Portability and Accountability Act, and Provider will not be responsible for the disclosure of any health information that constitutes protected health information (“PHI”) through your use of the messaging system. If you use, provide, transmit, store, or disclose PHI in connection with the Program, you agree to indemnify Provider against any damages, losses, liabilities, judgments, costs, or expenses (including reasonable attorney fees and costs) arising out such use, provision, transmission, storage, or disclosure of such PHI.
